How This Referral Guide Works

This guide is organised by NDIS registration groups, not individual service types. Each category page shows all providers registered under that group in a specific suburb, with NDIS line item codes, current price guide rates, and referral tips.

Why registration groups instead of service names?

  • Registration determines compliance. Whether a provider can legally deliver services to an agency-managed participant depends on their registration group, not their marketing.
  • Plans are structured by category. When you read a participant's plan, funding is allocated by support category. These pages map directly to how plans are built.
  • Broader provider discovery. A "Therapeutic Supports" search shows OTs, physios, psychologists, and speech pathologists together, so you can compare options across disciplines for the same participant.

How to make a referral

  1. Select a registration group above that matches the participant's plan funding.
  2. Choose the suburb where your participant needs services.
  3. Review the provider list, registration status, and pricing.
  4. Contact providers directly, or use the referral form to have us match providers for you.
  5. Set up a service agreement using the line item codes shown on each page.

What information is on each page?

Every suburb page includes the top 10 registered providers for that registration group, a full pricing table with NDIS line item codes, a step-by-step referral process, what to check before referring (registration status, availability, cultural needs, cancellation policy), and answers to common questions. The sidebar shows current price guide rates with links to individual line item pages.

Support Coordinator Resources

Frequently Asked Questions

How do I use this referral guide as a support coordinator?

Select a registration group category above, then choose the suburb where your participant needs services. Each page shows NDIS registered providers in that area with line item codes, pricing, and referral tips. You can also use the referral form to have us match providers for you.

Can I refer NDIA-managed participants to any provider listed here?

NDIA-managed participants can only use NDIS registered providers. All providers listed show their registration status. Plan-managed and self-managed participants can use both registered and unregistered providers.

Is this referral service free for support coordinators?

Yes. Browsing the directory, viewing provider details, and submitting referrals through MD Home Care is completely free for support coordinators. We connect you with providers at no cost.

What information do I need to make a referral?

To make an effective referral, have the participant's plan management type (NDIA, plan-managed, or self-managed), the relevant NDIS support category and budget, the suburb where services are needed, how urgently the participant needs the service, and any specific requirements (cultural, language, gender preference, accessibility needs).

How are providers listed and ranked?

Providers are filtered by NDIS registration group and suburb. They are ranked by a combination of NDIS registration status, profile completeness, and engagement with the platform. Listings rotate daily to give all providers fair visibility. Provider data is sourced from the NDIS provider register and verified by MD Home Care.
