What is the Transition Care Programme?

The TCP is a joint Australian Government and state/territory programme designed for older people who:

  • Have been in hospital and need more time and support before they can safely live at home

  • Have the potential to improve their functional capacity with the right short-term support

Transition care is not ongoing aged care. It is a time-limited programme (typically up to 12 weeks) focused on recovery and regaining independence.

What Does Transition Care Include?

  • Allied health therapies including physiotherapy, occupational therapy, and speech pathology

How to Access Transition Care

  2. Approval. If approved, your hospital team arranges a transition care place.
  3. Care delivery. You receive transition care either at home or in a residential facility, depending on your needs and what is available.
  4. Goal-focused recovery. Your care plan is reviewed regularly, with the aim of returning you to independent living.

Transition Care Settings

  • Home-based transition care. You return to your own home in Australia and receive visits from therapists, nurses, and support workers. This is the preferred option for people with adequate home support and a safe living environment.
  • Residential transition care. You stay in a dedicated transition care bed at a residential aged care facility while receiving intensive rehabilitation. This option suits people who need 24-hour support during their recovery.

After Transition Care

When your transition care ends, you may:

  • Return to living independently at home
  • Move to a Home Care Package for ongoing support
  • Access the Support at Home programme
  • Enter residential aged care if independent living is no longer safe
