About Support Coordination

Support Coordination helps participants implement their NDIS plan, connect with providers, and build capacity to manage their own supports. Level 1 (Support Connection) provides basic linkage. Level 2 (Coordination of Supports) is the standard level for most participants. Level 3 (Specialist Support Coordination) is for complex cases involving multiple services, high risk, or crisis situations.

With 33 providers across all states and territories, support coordination is a specialist registration group. Provider availability varies by location, so check suburb-level results for accurate coverage.

Support Coordination Pricing

The following NDIS price limits apply under the 2025-26 NDIS Pricing Arrangements. These are maximum rates for standard metropolitan areas. Remote and very remote loadings apply in eligible areas.

What Support Coordination Covers

The Support Coordination registration group includes the following services. When referring, match the specific service type to the participant's plan goals and funded line items.

Level 1: Support Connection

Basic linkage to services and supports. Helps participants understand their plan and connect with providers. Lowest level, suitable for participants with straightforward needs and good informal supports.

Level 2: Coordination of Supports

Standard support coordination. Plan implementation, provider research, service agreement management, progress reporting, and plan review preparation. The most common level, suitable for most participants.

Level 3: Specialist Support Coordination

For participants with complex, high-risk needs. Involves multiple interacting services, crisis management, justice system involvement, homelessness risk, or hospital discharge coordination. Higher rate reflects the specialist skills required.

Real-world situations where you would refer a participant for support coordination.

A participant's current Level 2 coordinator is leaving and they need a replacement. When referring, provide a handover document covering active providers, pending referrals, and plan review dates.

A participant with complex psychosocial needs and justice system involvement needs Level 3 specialist coordination. Refer to a coordinator with specific experience in the intersection of disability and criminal justice.

A participant is unhappy with their coordinator and wants to switch. Help them understand their right to change, then refer to an alternative coordinator. Ensure a proper handover period where both coordinators overlap for continuity.

What to Expect After Referral

When a participant transitions to a new coordinator, expect a 2-4 week handover period. The outgoing coordinator should provide a written handover document, and the incoming coordinator should contact all active providers to introduce themselves. Good coordinators are responsive (reply within 1 business day), proactive about plan utilisation, and transparent about their caseload capacity.
