It gives older Australians in Australia up to 12 weeks of concentrated allied health support, backed by approximately $6,000 in government funding, to rebuild strength, regain skills, and restore the independence that a health event has taken away.

This is not ongoing aged care. It is a focused burst of support with a clear goal: to get you back to where you were, or as close to it as possible, so you can continue living at home on your own terms.

What the Restorative Care Pathway Actually Is

The Restorative Care Pathway sits within the Support at Home program as a distinct short-term pathway. It provides funding for a tailored program of allied health services over a maximum of 12 weeks. A team of health professionals works with you to identify what you have lost, set realistic goals, and build a therapy program to help you get there.

What makes it different from ongoing aged care support:

  • Time-limited: Up to 12 weeks, not indefinitely.
  • Goal-oriented: Every session targets specific, measurable goals agreed with you at the start.
  • Intensive: More frequent sessions than typical ongoing support, because the aim is recovery rather than maintenance.
  • Allied health focused: Active therapy and rehabilitation, not passive care.

At the end of the 12 weeks, you are reassessed. Many people recover enough to manage without further support. Others transition to ongoing Support at Home services if they still need help.

What Is Included in the Program

The specific services you receive depend on your individual needs and goals. A restorative care plan is built around you, not around a standard template.

Physiotherapy

Physiotherapists work on strength, balance, mobility, and pain management. If you have had a fall, they will assess your risk factors and build an exercise program to reduce the chance of it happening again. After surgery, they help you regain range of motion and functional movement. Sessions happen in your home in Australia, using your actual environment, which means the exercises are practical and directly relevant to your daily life.

Occupational Therapy

Weeks 2 to 10: Active Therapy and Support

You attend regular therapy sessions, typically two to three times per week, in your home in Australia. Practising in your own environment means the exercises are directly relevant to your daily life. Your therapists also give you activities to do between sessions, and the program is adjusted based on your progress.

Weeks 10 to 12: Review and Transition
