Why service agreements matter in the NDIS

A service agreement sets expectations between a participant and a provider. For registered providers it is part of how you show participants what they are buying, how to give notice, and how complaints work. A clear agreement reduces disputes and helps auditors see that informed consent was sought.

What this template covers

The builder merges your form fields into a Word document with parties, service description, indicative pricing language, payment terms shaped by plan management type, and generic references to NDIS Practice Standards. It does not know your internal risk register, travel policy, or supervision model. You should add those where your organisation requires them.

Plan managed compared with self managed invoicing

Plan managed participants usually need invoices addressed to the plan manager with the right participant and plan identifiers. Self managed participants may pay you directly and claim from the NDIS portal, so your agreement should spell out who receives invoices and typical payment days. This tool adds a short payment line based on your selection; refine it with your finance team.

Price guide rates and line items

Support coordination has several registration groups and line items. The hourly figures in this tool are common examples for planning conversations only. Before you quote a participant, confirm the item code, travel rules, and price limits that apply on the service date.

How MD Home Care fits in

MD Home Care helps participants and families discover and compare registered providers. It does not employ support coordinators to deliver services under this agreement template. Use the directory when you are ready to connect participants with vetted providers in your network.

Frequently asked questions

Do I need a written service agreement for NDIS supports?

Registered NDIS providers must have service agreements with participants for funded supports in most circumstances. Agreements set out services, fees, cancellation rules, and rights. This builder produces a starting document only; your organisation should align it with your policies and legal advice.

Are the hourly rates in this tool guaranteed?

The tool uses example rates that match common support coordination line items in the NDIS price guide for planning purposes. Published limits change with NDIS updates. Always confirm the current price guide and line item codes before invoicing.

Is the downloaded file legal advice?

No. The output is a structured template for you to edit. It does not replace advice from a lawyer or your own quality and safeguarding policies. Participants and providers should read and negotiate terms before signing.

What is the difference between plan managed and self managed in an agreement?

Plan managed means invoices usually go to a plan manager for payment from NDIS funding. Self managed means the participant (or nominee) pays invoices and claims reimbursement, so payment terms and who receives invoices must be clear in the agreement.

Does MD Home Care sign or store my agreement?

No. Generation happens in your browser session. MD Home Care is a connection platform and does not provide support coordination or store your DOCX. You are responsible for secure storage and signatures.

Need more NDIS providers on your radar?

Search vetted registered providers by service and location. MD Home Care connects participants and families with options; agreements stay between you and your client.
